Complete the enrollment process by enrolling in My Chart/My Nm or by contacting our office and completing documents using Docusign.

How to begin your subscription. We’ll need to learn more about you so we’ll ask some questions about your health during the onboarding process to best tailor your care.
After you are registered and enrolled, you can start using MyChart to communicate with our providers and manage your health.

Once you have set up MyChart, you will be able to schedule appointments in the system or by calling and speaking to a live operator.

Treatment
MyHealthCare Option™ offers refills for chronic maintenance prescriptions.

Maintenance prescriptions for chronic medical conditions are available to subscribers with a subscription.

To obtain most prescriptions with MyHealthCare Option™, you will first need to complete labs so we can safely tailor treatment for you.

We’ll need your lab work results on file which can be sent through EPIC to our office.

Either the business or the employee can pay the monthly fee, and the plan is FSA eligible. If the employee wants to add family members, that cost is their responsibility. The employee also pays the $9.99 copay each time they use the service.
Your subscription includes on-demand prescriptions, maintenance prescriptions, preventive care guidelines, cancer screenings, weight loss management, sick/on-demand messaging or audio televisits, and chronic disease management, and more.
Employees can access care through MyNM MyChart, where they can schedule virtual visits, request eVisits, send messages to doctors, and receive preventive care services. The platform ensures convenient, secure, and timely access to medical attention from your phone, laptop or desktop.
Employees sign a DocuSign agreement. If the employee is covering the subscription cost, the employee sets up payment either with a credit card or ACH bank deposit. They need to access MyChart to enter their medical information and demographics. They then can schedule appointments through Calendly after paying a $9.99 copay per visit.
Employers sign a contract and set up payment through ACH payment through their bank. They can then add or remove employees monthly. New patients complete enrollment using Docusign, answer health history questions, and schedule appointments or eVisits through MyChart.
Patients must have pertinent lab studies performed at least within 6 months of enrollment and then yearly. This lab work must be sent to or made available through EPIC for review before issuing a prescription. Follow-up blood work may be required for prescription changes.
Yes, at the provider’s discretion. Medications may be changed, and authorizations may be completed depending on the insurance carrier. Patients may need to discuss alternative arrangements with their physicians.
MyHealthCare Option provides comprehensive preventive care services upon request, including Immunizations recommendations, cancer screenings, and routine check-ups. These services are designed to promote early detection and long-term health. If vaccinations are recommended, MHCO sends that prescription to the pharmacy of your choice.
We generally recommend using either GoodRx or Walmart's discount program. If you already have medical insurance including state plans/public aid, we recommend keeping that insurance because it will give you a discounted price on your prescriptions.
First the employee or individual enrolls and their subscription is paid for. A Welcome Letter is emailed to confirm enrollment along with a link to a Docusign envelope for the dependents enrollment which will connect them to your account. You are responsible to pay for your dependents subscriptions.
